Managing diabetes requires a new standard of care and a personalized approach. One where the financial burden of speaking with a clinician and counting strips and supplies is minimized. Perry’s services are covered by Medicare Part B. This means that during the public health emergency, all copays are waived so there is no cost to you for the program, our smart connected meter and testing supplies delivered to your door
If you are interested in Perry Health’s services, you can fill out this form or call us at 866-447-3779 and one of our patient enrollment specialists will ask you a few questions about your medical history to determine if you qualify. The process doesn’t take more than 10 minutes, and the enrollment specialists can answer any questions you may have.
Your Enrollment Specialist will schedule you for an introduction with your Accountability Coach, who will further gather some additional information about your medical history and determine your goals. Your Coach will help to schedule various consultations with our clinical team and dietitians, who will help guide you on your journey to lower you A1c. You'll meet with your coach a couple times a month for 10-15 minutes, as well as have structured 30 minute meetings with our clinical and dietary team. We'll also ship out your cellular connected Blood Glucose Meter and strips and supplies will be regularly delivered to you. If readings ever trigger an alert in our system, a member of the clinical team will reach out immediately.
Due to the Public Health Emergency as a result of the COVID-19 Pandemic, there are no out of pocket costs for Perry Health if you are on an eligible Medicare or Medicare Advantage plan. Give us a call to learn more.
